National Institute of Environmental Health Sciences Seminar Topic: Bridging Micro-Analysis and Mac
Abstract: Chronic respiratory diseases, such as Chronic Obstructive Pulmonary Disease (COPD) and non-cystic fibrosis bronchiectasis, are highly heterogeneous conditions that require individualized care. Our nationwide cohort data highlight East Asian-specific characteristics, such as a high prevalence of Klebsiella pneumoniae and Pseudomonas aeruginosa colonization, which strongly correlate with severe exacerbations and higher mortality rates. We also utilize the validated ROSE criteria to identify the overlapping COPD-bronchiectasis association. Clinically, we refine patient management through tailored functional and therapeutic assessments. Novel functional indices, such as pace variability during the 6-minute walk test (6MWT) and the BHDE-index (incorporating post-exercise heart rate recovery), effectively stratify mortality and hospitalization risks. We also employ the remote dielectric sensing (ReDS) system to measure dynamic lung fluid changes during the 6MWT, which helps distinguish overlapping heart failure in COPD patients. Therapeutically, implementing Peak Inspiratory Flow Rate (PIFR)-guided inhalation therapy significantly reduces severe exacerbations by ensuring optimal, individualized drug delivery.
